    Summary of WorkSafe Inspection – 112 Ellscott Blvd, Mickleham

    Inspectors attended site following a complaint about incorrect concrete slump causing a pump clamp failure and injury.
    No concrete works or pumps were present during the visit.
    Previous works involved pile pouring using a line pump.
    After the final pour, site management discussed slump adequacy with the pump subcontractor and instructed:

    Stop work immediately and notify management if slump concerns arise.


    Remaining piles were poured directly from agitators (no line pump used).
    Site management reported no incidents or clamp failures during line pump use.
    Internal incident register (Hammertech) showed no logged incidents.
    All pile works are now complete.
    Future slab pours will use a boom pump, with inductions reinforcing:

    Cease work and notify management if slump concerns occur.




    Recommendations
    Please note that adding water to concrete on site is not permitted under Australian Standards:


    AS 1379 – Specification and Supply of Concrete
    Clause 7.2.2: Water shall not be added after batching except as permitted and only to achieve specified slump without exceeding the maximum water–cement ratio.


    AS 3600 – Concrete Structures
    Water–cement ratio is critical for strength and durability. Increasing water beyond design mix reduces compressive strength and may result in non-compliance.


    NCC Part 3.2.3(b):
    Water must not be added to increase slump beyond specified value.


    Impact:
    Adding ~10L water per m³ can reduce 28-day compressive strength by ~2.5 MPa, compromising structural integrity.
    Correct Procedure:
    If a different slump is required, request this at the batching plant so adjustments can be made using approved admixtures, not water.

  • 27th octubre 2025, TOOLBOX TALK – PILING PLATFORM SAFETY & MAINTENANCE

    Purpose
    To ensure the piling platform remains safe and compliant to support the Mait HR120 piling rig during operations.
    The platform has been assessed as suitable, but only if it is maintained correctly throughout the piling works.

    Key Points
    1. Surface & Drainage
    • Platform must remain firm, level, and well-drained.
    • Maintain 1–2% slope to prevent water ponding.
    • Ensure drains and runoff paths are clear at all times.
    • Report any pooling water or soft areas immediately.

    2. After Rain or Water Exposure
    • Rain can weaken the clay subgrade and reduce bearing strength.
    • Re-inspection is mandatory after rainfall over 20 mm in 24 hours.
    • Any soft or wet zones must be:
    ○ Excavated out,
    ○ Backfilled with compacted granular material (min 8 passes), and
    ○ Rechecked before restarting piling.

    3. Surface Condition
    • Regularly inspect for ruts, settlement, or cracking caused by rig movements.
    • Any damage must be reinstated to the original thickness and re-compacted before continuing.
    • Keep the platform free of spoil, mud, or obstructions that could affect stability.

    4. Edge Protection
    • Maintain a minimum 2 m setback from all batters, trenches, pits, or vertical edges.
    • Unrestrained edges to be trimmed to a 45° slope.
    • Do not position the rig near unsupported or soft edges without engineer approval.

    5. Monitoring & Reporting
    • Conduct daily inspections and log observations.
    • Stop work immediately if any ground movement, rutting, or instability is noticed.
    • Notify the Supervisor or Geotechnical Engineer (DTM Geocivil) if distress signs are observed.

    Remember
    ✅ Safe platform = safe rig operation
    ✅ Drainage and maintenance are key
    ✅ Stop work if in doubt – inspect and confirm before continuing
